AbstractPurpose: This study examines the management strategies, legal frameworks, and international patient care protocols that underpin the global medical and wellness tourism industry. As healthcare becomes increasingly borderless, the research aims to identify the critical success factors and regulatory gaps that influence patient safety and institutional competitiveness in a globalized market. Methodology: Following the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines, a systematic literature review was conducted across major academic databases, including Scopus, PubMed, and Web of Science. The study synthesized data from 65 peer-reviewed articles and policy reports published between 2010 and 2024, focusing on the intersection of healthcare management, international law, and service quality models (SERVQUAL). Findings: The results indicate that international accreditation (e.g., JCI) increases patient volume by 35% by serving as a primary trust-building mechanism. However, a significant "accountability gap" exists, with fewer than 10% of international malpractice claims resulting in successful redress due to jurisdictional complexities. Furthermore, the integration of digital health platforms and culturally competent care protocols was found to reduce clinical errors by 40% and improve patient adherence to post-operative recovery plans. The study also identifies an ethical "dual-sector" effect, where medical tourism can exacerbate local health inequities through specialist "brain drain." Practical Implications: For hospital administrators and policymakers, the study suggests that long-term sustainability depends on shifting from a costreduction model to a high-value "continuum of care" model. This includes establishing bilateral health treaties, implementing standardized "virtual patient journeys" via telemedicine, and developing "social return on investment" strategies to ensure local health systems benefit from international commerce. Originality/Value: This research contributes to the field by providing an integrated framework that bridges the gap between clinical protocols and legal governance. It offers a comprehensive perspective on how technology and cultural sensitivity function not merely as marketing tools, but as essential clinical safety components in the 21stcentury health tourism ecosystem.
